ACCEPTING NEW TELEHEALTH CLIENTS Welcome to THE PATTON PRACTICE Theresa Patton, CRC, LCPC, LPC, LCAC, LCAS, LIAC, LCDC, MAC, CCS I treat adult clients via TELEHEALTH in AZ, IL, IN, MO, NC, and WI The Practice is committed to providing mental health, addiction, and rehabilitation counseling. I offer clinical support for behavioral healthcare and assistance with both short- and long-term disabilities. Our goal is to help you regain your health and function. Are you seeking support during a life transition? Have you recently experienced an injury or need emotional assistance? Are you looking to re-enter the workforce or acquire new job skills? I am here to help. I am a licensed mental health, addiction, and rehabilitation professional who specializes in helping individuals identify and achieve their goals. THE CLIENTS SERVED: The Patton Practice serves adults, with a particular emphasis on older adults, including those with geriatric needs. We welcome individuals of all genders and sexual orientations and wholeheartedly support LGBTQIA clients, as well as those with emotional or physical disabilities, including individuals facing substance use and mental health disorders. We aim to help you overcome your challenges by providing thorough evaluations and developing personalized treatment and development plans. These plans may also include strategies to help you return to work. THE PROCESS: Your appointment is an important time set aside just for you, whether it is for an emergency or a scheduled clinical session. I will ask for your feedback to make sure we are aligned. During our sessions, I will use evidence-based best practices that are supported by comprehensive research and proven results. THE WORK WE WILL DO TOGETHER: I will challenge you as you support the changes and goals you set for yourself. As your advocate and greatest supporter, I am committed to helping you heal both your body and mind. My approach is professional, warm, and personal. I will recognize and address your needs with respect and professionalism. I will use available resources to support you during your transition, whether that means returning to work or reconnecting with family. Together, we will tackle your personal, physical, and social challenges or disabilities. After each visit, I will continue to connect the dots to help you gain insight and clarity. FINANCIAL POLICIES: As a private practitioner, I prioritize your treatment needs to ensure continuous care. I am in-network with Aetna, Blue Cross Blue Shield of Illinois (BCBSIL), Anthem, and Anthem Employee Assistance Program (EAP). If you are out-of-network, I will automatically generate invoices in your client portal and submit them promptly to your insurance company for reimbursement, in accordance with your policy terms. If you cancel your appointment with less than 24 hours' notice, we will charge the full session fee to the credit or debit card on file. To avoid being charged the full session fee, submit cancellations through a secure portal, via text, or by phone at least 24 hours before the appointment. Failure to cancel may impact your ability to book future appointments. If we cannot charge the card on file for any reason, such as a declined or expired card, the client's current appointment will be canceled. You can reschedule the appointment once you update your billing information through the client portal. THE CLIENT PORTAL: Active clients can access our HIPAA-compliant communication portal. Telehealth sessions are securely encrypted and HIPAA-compliant, ensuring your information remains safe and confidential. FINAL THOUGHTS: Therapy is a commitment that promotes self-awareness and personal growth. To maintain your active client status, you must schedule at least one appointment per month unless you have made alternative arrangements with your clinician in advance. If you do not schedule or attend an appointment within thirty days of your last appointment, your portal access will be deactivated, and you will be discharged to inactive status. This will prevent you from scheduling future appointments. If you have two or more cancellations within any 60 days while you are an active client, your status may be changed to inactive. This could result in the cancellation of any upcoming appointments and a potential discharge from the practice. Additionally, we may deactivate your portal access, and we may restrict your ability to make future appointments. Counseling can provide stability and significantly enhance your overall well-being. It requires a committed partnership between you and your therapist. By choosing THE PATTON PRACTICE, you are investing in your most valuable asset: yourself. I look forward to your visit.
